Community Connections is a nonprofit serving Southwest Colorado, helping people with disabilities, developmental delays, and complex needs access the services and support they need to live healthy, independent lives. Through Medicaid case management, early childhood programs, and family support services, Community Connections connects individuals and families to essential resources, guiding them through complex systems of care and ensuring they can thrive in their homes and communities.
